Home 4Ever Community Services has partnered with Trilogy Care, an approved Support at Home provider, to ensure you have access to high-quality home care services via your Support at Home funding. Trilogy Care can administer your care package while you have the flexibility to work with the service providers of your choice.
Both organisations are focused on empowering you to maximise your care hours and wellbeing by giving you choice, control and independence over your care.
Trilogy Care’s model empowers you to have the flexibility to choose your service providers and gain more control over how your Support at Home budget is spent. This means you can continue to work with Home 4Ever Community Services and have Home 4Ever Community Services support services funded under your Support at Home budget – subject to your care plan and budget limits.
Working with Home 4Ever Community Services and Trilogy Care means you will spend less on Support at Home management fees so you can spend more of your budget on the things that matter – care and services to help you stay at home longer.
Who is Trilogy Care
Trilogy Care supports older Australians to live well at home for longer, by helping them build the Support at Home funding they want and need.
What is Support at Home?
Support at Home is a funding service provided by the Australian Government that supports older Australians to live safely and independently in their own home.
The program is designed to support you with daily activities your clients may need assistance with, such as cleaning, showering, or attending appointments.
All Support at Home clients need an approved provider, such as Trilogy Care, to administer the Government subsidy.
